How erection maintenance works
An erection depends on blood flow, nerve signals, hormone balance, arousal, confidence and the ability of penile veins to hold blood inside the erectile tissue. Some men can get an erection during foreplay but lose firmness during penetration because one or more of these systems is under strain.
Psychological and performance-related causes
Performance pressure, fear of failure, relationship stress, previous disappointing experiences and anxiety can interrupt arousal signals. This does not mean the problem is imaginary; the mind and body are connected in sexual function.
Diabetes and vascular disease
Diabetes, high blood pressure, smoking, high cholesterol and obesity can affect penile blood vessels and nerves. Difficulty maintaining an erection can sometimes be an early warning sign of metabolic or cardiovascular risk.
Tests and treatment approach
A doctor may review sugar levels, blood pressure, lipids, testosterone, medicines, sleep, lifestyle and relationship factors. Treatment is most effective when it targets the cause rather than offering a temporary power booster only.
